To ensure the safety and security of our community, all vendors must provide the required information and documents before being granted access to the property. This form must be completed and submitted along with valid copies of the vendor's license and insurance.The vendor's insurance must list Doral Gardens Condominium Association as the certificate holder and, most importantly, as additional insured. This requirement is essential to safeguard the interests of the community.

Terms and Acknowledgment

By submitting this form, the vendor and the resident/owner requesting access acknowledge and agree to the following terms:

Responsibility for Damages:
The vendor and/or owner is solely responsible for any damages caused to the property, including but not limited to common areas, during the performance of their work.

Condition of Common Areas:
The vendor must leave all common areas in the same condition as they were found. Failure to do so may result in additional charges or denial of future access.

Compliance with Rules:
The vendor must comply with all community rules and regulations while on the property. Work is only allowed Monday-Saturday from 8am to 5pm.

Insurance Requirements:
Access will be denied if the required documentation listing Doral Gardens as both certificate holder and additional insured is not provided.

All requests submitted are reviewed the same day, provided they are received before 3:00 PM. You will receive an email notification once your request has been reviewed, indicating whether it has been approved or not. For any questions, please call 786-577-2976.
